Holland Police Department general incident report 940308035 (UFO sightings, 8 March 1994)
The Holland, Michigan, police report on the sightings of the night of 8 March 1994, written by Officer Jeff Velthouse. It records his own observation through binoculars, other witnesses' accounts, and his conversation with National Weather Service meteorologist Jack Bushong about radar returns. The report records what the witnesses said, not what the lights were.

- documented Officer Jeff Velthouse (badge 73) responded to UFO calls in Holland, Michigan, on 8 March 1994; 911 dispatch had received a number of calls. Through a resident's binoculars he saw an object high to the southwest with red, green and white lights but could not make out a shape. He wrote that the lighting "appeared to be different than a normal aircraft" and was "oscillating, rather than blinking", and that he saw two objects moving southwest. - documented Velthouse asked dispatch to call the National Weather Service in Muskegon, which reported tracking three objects heading southwest between Holland and Benton Harbor. He drove to Tulip City airport but could not relocate them. Source: doc-holland-police-report-1994.pdf
- documented Later that shift he spoke with Jack Bushong of the National Weather Service, who said he had three returns on his radar, larger than normal aircraft, in a triangular shape. One separated from the triangle and returned and another separated. They were at 15,000–25,000 ft, and one moved about 20 miles from the South Haven area over Lake Michigan in roughly 10 seconds.
